Residents may visit our offices or call and email our office and ask for an affidavit form by mail. The form must be signed, notarized, and returned to us by mail. Real estate property records do not need to be notarized.
All RR/CC locations (except Beverly Hills Branch) can accept and process applications for vital records. Only LAX, Van Nuys, and Lancaster offices can accept and process requests for real property. For a list of locations, click here.

Only immediate family members (parents, children, grandparents, grandchildren, and siblings) can request official certified copies for vital records.

Processing and Delivery Timeline:
- Same-day in-person delivery for most recent records
- Records older than 1962 and records between 1972 and 1977 will be mailed out within two (2) business days—these records require additional processing.
NOTE: Some birth records have parent social security numbers on the birth certificate. We are required to redact that information because it is personally identifiable information.
We only hold records from L.A. County, but we can help people obtain vital records from other counties in California.
Local Assistance Centers
We are working closely with county departments and organizations to provide comprehensive support at Local Assistance Centers. Our staff are present to assist with requests for vital records (birth, death, marriage certificates), property records, and business filings.
