PLEASE READ BEFORE PREPARING THE STATEMENT OF DOMESTIC PARTNERSHIP

This office provides a place to publicly register your relationship by filing a Statement of Domestic Partnership.

You are not required to check any of the optional boxes. Whichever you choose to check are between you and your partner.

The Statement of Domestic Partnership may be accepted by your employer for other purposes such as eligibility for health insurance.

Eligibility requirements for Domestic Partnership benefits are determined between you and your employer.

A STATEMENT OF DOMESTIC PARTNERSHIP DOES NOT TAKE THE PLACE OF THE PROPERLY EXECUTED LEGAL DOCUMENTS WHICH YOU MUST HAVE IF YOU WISH TO AUTHORIZE EACH OTHER TO MAKE MEDICAL OR FINANCIAL DECISIONS, LEAVE PROPERTY TO EACH OTHER OR MAKE OTHER ARRANGEMENTS CONCERNING INCAPACITY OR DEATH. SUCH DOCUMENTS CAN PROVIDE CRITICAL PROTECTION FOR YOU AND YOUR PARTNER IN THE CASE OF MEDICAL OR OTHER EMERGENCY. IT IS IMPORTANT TO CONSULT WITH AN ATTORNEY FOR ASSISTANCE IN PREPARING DOCUMENTS THAT ARE APPROPRIATE FOR YOUR NEEDS. THE STAFF OF THE REGISTRAR-RECORDER/COUNTY CLERK IS NOT AUTHORIZED TO PROVIDE LEGAL ADVICE. THE STATEMENT OF DOMESTIC PARTNERSHIP IS A PUBLIC RECORD.

If you and your partner would like to file for a domestic partnership, take the following steps:

Make sure you read the important domestic partnership informational document above
Make sure you and your partner meet the requirements
1. You must both be 18 years or older
2. At least one of you must live or work in Los Angeles County
Complete the Statement of Domestic Partnership Form and bring it to the office
