Request a Copy of a UCC Financing Statement Record by Fax

Fax the following information to (562) 864-1250:

  • Title of the document.
  • Names listed on the document.
  • Year the document was recorded.
  • Credit card payment information:
    • Name on the credit card.
    • Credit card number.
    • Credit card expiration date.
  • The address where you would like the record mailed.
  • Whether you would like it sent by regular mail or Express Mail (UPS).
  • A daytime phone number, including area code.

When Will I Receive My Copy?

The copy of the UCC financing statement document(s) requested will be mailed within five business days after the request is received. A "No Record Statement" will be issued if no record is found.
