File a Fictitious Business Name Statement 

Who Can File

  • Persons doing business for profit under a fictitious name (does not include surname in company name)
  • Statement must be filed in county of principal place of business and may also be filed in other counties as long as the requirements for filing in the county of principal place of business have been met
  • Non-profit corporations, organizations or associations are not required to file a Fictitious Business Name Statement

NEW Online Filing

Registered business owners (registrants) can now file their Fictitious Business Name (FBN) completely online! Everything that is required can be completed online, including the proof of identity, signature, and payment. Filing online includes a $9 service fee.

Once the filing is completed, the registrant will receive the FBN filing by email.

In-Person Appointment

Appointments can be scheduled up to three (3) weeks in advance.

Before you can make an appointment you must complete the Online Business Filing and Registration System. When you complete the application and receive your confirmation number you can then make an appointment.

Mail Request

Mail filings are only processed at our Norwalk Headquarters. District offices do not process mail requests.

All mail requests must include a completed Fictitious Business Name Statement and a notarized Affidavit of Identity form.
