Certified copies of Fictitious Business Name Statements (at $2 per copy) are available by appointment.

Mail Requests

Mail requests are only fulfilled by the Norwalk Office. Please mail in your request and check (payable to the Registrar-Recorder/County Clerk) to the Norwalk address provided below:

Phone Requests

To receive a copy of a Fictitious Business Name Statement over the phone, please contact (800) 201-8999, and provide:

• FBN Statement information
• Credit card information

Once everything is completed a copy will be mailed out to the address provided.
